Hazards: There is risk of serious injury or death from electric shock. What to do: Stop using the product immediately. Contact Direct To Retail to: make arrangements to get a replacement product in exchange for the affected adaptor; or get an authorisation number to allow you to return the adaptor to a Chemist Warehouse store for a full refund.
