Reason for Recall / Hazard

The Karma Flexx Wheelchair may have been supplied without the yellow labels that indicate the correct location of the anchor points for in-vehicle mounting. These labels are required to comply with the requirements of AS/NZS3696.19:2009 - Wheelchairs Wheeled mobility devices for use as seats in motor vehicles. The user manual supplied for the Karma Flexx TALL wheelchair (models KM-8022/8522 TL) fails to indicate that the chair has been tested according to the standard and is suitable for use as a seat in a vehicle.

Product Description & Identification

Reason / Defects: The Karma Flexx Wheelchair may have been supplied without the yellow labels that indicate the correct location of the anchor points for in-vehicle mounting. These labels are required to comply with the requirements of AS/NZS3696.19:2009 - Wheelchairs Wheeled mobility devices for use as seats in motor vehicles. The user manual supplied for the Karma Flexx TALL wheelchair (models KM-8022/8522 TL) fails to indicate that the chair has been tested according to the standard and is suitable for use as a seat in a vehicle. Hazards: If a user places the mounting straps in an incorrect location on the wheelchair frame, there is a risk of the chair moving or falling in the event of an accident, increasing the risk of a serious injury, permanent disability and/or death. What to do: Fisher Lane will contact affected consumers in writing to arrange to provide a repair kit that will include a new user manual, labels and instructions. Consumers should take the following actions: 1. Inspect the wheelchair and determine if the correct labels are present or not. 2. If the labels are absent, attach the four labels to the frame of the wheelchair following the instructions that have been provided. 3. Discard the older user instruction manual and replace it with the new instruction manual. 4. Complete the Reply-paid card and return it to Fisher Lane.
