Reason / Defects: The casing of the adaptor can develop a crack which can expose the live electrical contacts inside. Hazards: There is a risk of electric shock, serious injury or death if internal electrical contacts are exposed. What to do: Consumers should stop using the adaptor immediately and disconnect the plug from the power outlet and the audio mixer. Consumers should contact Yamaha via the online portal to arrange a free replacement. For more information, consumers can contact Yamaha on 1800 331 130 (select option 1) or by emailing mpdorders_aus@music.yamaha.com

Only adaptors numbered with a date code between 2126 – 2319 are recalled. The date code is inscribed on of the back of the adaptor.
